We have set-up a service which lets you order repeats and book appointments on-line.

This service requires registration: you can download a PDF registration document here - or you can pick one up at reception
Please return this form to the surgery. You will be able to collect your registration letter with your personal security PIN number from reception after five working days.
Please note - you will be required to show some form of identification before your details can be released.
After you have received your registration details you will need to register on-line by accessing the system here
If you have not already done so, please collect your registration details from the surgery, so that you can use the system.
